This item increases armor penetration, negating damage-reducing armor on enemies. This is most useful with enemies who have high levels of armor, such as bosses. It's a good match for builds that rely on high rates of attack but lower damage, especially late game. Without Bone Hilt, those builds become increasingly less powerful because each hit is reduced by an enemy's armor value (currently 0.5HP per point of armor).
